The Hama?elas Community Kitchen Project is dedicated to creating a warm and culturally safe environment where hot dinner meals are served every day of the year. The objective of this project is to reduce barriers experienced by individuals facing homelessness or housing insecurity by providing consistent access to nutritious meals, fostering social connection, and increasing access to essential community resources and supports. Through a partnership with the Salvation Army in Campbell River, Laichwiltach Family Life Society will provide one hot dinner meal Monday through Friday, creating a welcoming and culturally safe environment that promotes dignity, well-being, and community engagement.
The project seeks to improve food security, strengthen community connections, and support individuals in accessing services that contribute to greater housing stability and overall wellness. In response to identified community needs, the Society's long-term goal is to expand meal service delivery to include Saturdays and Sundays, ensuring increased access to nutritious meals and supportive services throughout the week.

The Comox Valley Transition Society project delivers a coordinated and comprehensive range of supports designed to address homelessness and housing instability. Through three part-time positions, a Case Worker, Shower Attendant, and PEERS Support Worker, the project integrates Housing Services, Prevention Diversion, and Client Services to provide seamless, person-centred care. The initiative emphasizes low-barrier access through a dedicated shower program and weekly outreach, creating consistent opportunities to engage individuals experiencing homelessness and connect them to essential supports.
Services include assistance with basic needs, administrative support, and access to health and mental health resources. The project also focuses on homelessness prevention by providing emergency financial assistance and early intervention supports to individuals at risk of housing loss. In addition, it supports housing attainment and stability through housing navigation, skill development, and guidance related to tenancy and independent living. Overall, the project is designed to reduce barriers to service, strengthen community connections, and support individuals in securing and maintaining stable housing while improving overall well-being.

To improve housing stability and self-sufficiency among individuals experiencing homelessness or housing instability in the Courtenay/Comox area through a coordinated, formalized access point and case management system. The Tenant Support Worker will assess individual needs, develop and implement collaborative case management plans, support clients in setting and achieving housing and wellness goals, and connect them to essential community services. Through direct support and enhanced coordination with partner agencies, the project aims to address core housing needs, reduce barriers to housing retention, and strengthen long-term housing outcomes for vulnerable individuals.

The project aims to support individuals experiencing or at risk of homelessness in Courtenay, B.C. by delivering coordinated, client-centred Housing Services, Prevention, and Client Support Services through a part-time Housing Coordinator and a part-time Case Manager funded by Reaching Home Rural and Remote. The Housing Services objective is to assist participants in securing and maintaining safe and stable housing through housing navigation, placement support, and ongoing housing retention services. The Prevention objective focuses on reducing the risk of homelessness by providing early intervention supports, including access to financial resources and subsidies, tenancy education, and advocacy with landlords and housing providers to help prevent eviction and housing loss. The Client Support Services objective is to strengthen long-term housing stability and overall wellbeing through individualized case management, transitional supports, system navigation, and advocacy that helps participants successfully maintain housing and improve their overall quality of life.

To provide targeted support to individuals and families experiencing homelessness by assessing their immediate needs and facilitating access to essential supports, including food, this project will operate a low-barrier Drop-In Hub in the Creston area offering a safe, welcoming, and accessible space. The Hub will provide daily meals and essential items such as clothing, hygiene products, outdoor gear, and harm reduction supplies. It will also reduce barriers through outreach supports, including transportation assistance and access to showers and laundry. In addition, the Hub will serve as a centralized access point, connecting participants to health, housing, income, and employment services through coordinated referrals and on-site supports.

Working with community partners, landlords, and tenants, the Coordinator will support access to safe housing, provide advocacy, and help maintain tenancies while also ensuring effective coordination of resources and data collection.

To improve the self-sufficiency and housing stability of individuals experiencing or at risk of homelessness in the Kitimat community through coordinated Housing Services, Prevention and Shelter Diversion, and Client Services. The project will operate through a defined access point, where intake, assessment, and service coordination begin, delivered by a case management team consisting of an Intake Support Worker and Housing Support Worker.
Through coordinated outreach, assessment, and service navigation, the team will work with clients to set and achieve individualized goals, address immediate housing needs, and support long-term stability. The project will enhance service coordination and provide direct, client-centered supports, including housing placement, tenancy sustainment, eviction prevention, and connections to income, mental health, and substance use services, with a focus on individuals facing complex barriers to housing.

The Nicola Valley Homelessness Support Project works to reduce homelessness by not only providing housing placement and shelter diversion services but also by offering supportive housing, life skills development, and connections to essential community resources. By addressing both immediate housing needs and the underlying factors that can threaten stability, the project empowers individuals experiencing, or at risk of, homelessness to secure and sustain safe, stable housing, improving their overall well-being and strengthening the community.

Greater Vancouver Youth Unlimited will utilize Reaching Home Rural and Remote funding to support three part-time Youth Workers, who serve as the primary access point for operating the “My House” refuge. This refuge provides a welcoming, low-barrier space where youth experiencing or at risk of homelessness can access immediate safety, stabilization, and essential supports, including food, showers, laundry, and wraparound assistance. These roles ensure consistent, relationship-based outreach, crisis intervention, and connection to housing, health, and community services, directly addressing the immediate conditions that contribute to and perpetuate homelessness.
Through this model, the project reduces homelessness by providing an alternative to unsafe street involvement, supporting early crisis intervention, and creating consistent engagement that strengthens pathways into housing stability, improved well-being, and long-term supports.

Island Crisis Care Society (ICCS) will utilize Reaching Home Rural and Remote funding to support an Outreach Worker responsible for coordinating and delivering client-centred services for individuals experiencing or at imminent risk of homelessness in Parksville. The Outreach Worker will operate a low-barrier Access Point, providing intake, assessment, and ongoing support to up to 60 individuals. This includes connecting clients to housing, health, income, and community-based services, while improving system navigation and reducing barriers to access.
The project will provide immediate stabilization supports such as food, hygiene supplies, clothing, gift cards, and emergency survival items, as well as assistance with benefits navigation and transportation to essential services, particularly in rural and underserved areas. Culturally safe engagement will be supported through connections to Indigenous-led and culturally appropriate services for Indigenous clients. For individuals transitioning into housing, move-in supports will be provided to support housing stability. Overall, the project aims to strengthen service coordination, improve access to essential supports, and reduce homelessness through targeted outreach and stabilization services.

Risebridge will provide low-barrier, 7-day-a-week access to housing and crisis supports for individuals experiencing or at risk of homelessness in Parksville and surrounding communities, including Qualicum, Whiskey Creek, Nanoose, Errington, Dashwood, and Bowser. The project will utilize the three Reaching Home work areas, Housing Services, Homelessness Prevention, and Client Services, to deliver an integrated, coordinated response across the homelessness continuum. Through an outreach-based model, the project will provide immediate supports including shelter access, treatment referrals, food and survival supplies, transportation, moving assistance, landlord advocacy, and emergency financial supports such as crisis stipends and deposits.
It will improve access to services across rural and remote communities by reducing barriers, strengthening system navigation, and increasing engagement. The project will also strengthen collaboration and service coordination through partnerships with frontline agencies and community organizations, in alignment with Reaching Home objectives. This includes shared programming and coordinated service delivery such as food programming, clothing and housing supply distribution, weekly community meals, cultural and arts-based activities, indoor community spaces, and access to essential survival items. This partnership-based approach aims to strengthen housing stability and reduce homelessness across the region.

A dedicated access point will be created in Port Alberni to enhance agency coordination and provide direct support for clients facing housing loss and require basic living needs. This initiative will include bi-weekly life skills workshops designed to equip individuals with essential skills for self-sufficiency. Furthermore, outreach and housing support services will be broadened across the Nuu-chah-nulth Nation corridor, delivering vital supplies to six communities. The program will also feature educational resources focused on preventing housing loss and fostering self-sufficiency through the weekly distribution of materials.

The objective of this project is to implement a Housing First–based service system to support individuals experiencing homelessness and housing insecurity. The project will provide case management and essential supports, expand access to housing through landlord engagement, and coordinate a multi-service team offering legal, financial, vocational, health, mental health, and peer supports. Through targeted case management, including brokering access to dental and mental health services, the initiative aims to promote housing stability, improve well-being, and foster long-term resilience in the community.

The objective of the Smithers Homelessness Prevention Program is to reduce and prevent homelessness by strengthening housing stability for individuals experiencing chronic or episodic homelessness, as well as those who are precariously housed or at risk. Through the Housing Coordinator (0.8 FTE), the project will provide individualized, client-centred support to help participants secure and maintain safe, appropriate, and permanent housing. The project aims to address core housing needs by assisting clients with tenancy skills, housing system navigation, and connection to income supports, health services, and community resources.
A key objective is to build and maintain ongoing supportive relationships with clients to promote long-term housing stability and self-sufficiency. In collaboration with local community partners, the project will coordinate housing solutions and respond to housing crises in Smithers and surrounding areas. Overall, the project seeks to reduce homelessness, improve housing retention, and support individuals in achieving sustained well-being and independence.

The objective of the Squamish Helping Hands Society (SHHS) Community Outreach Project is to establish a coordinated, formalized access point for individuals experiencing or at risk of homelessness to receive timely, person-centred support. The Outreach Worker will engage individuals to assess needs, develop individualized case plans, and support goal setting focused on housing stability and well-being. The project will strengthen connections to essential health, housing, and social services, while providing direct outreach supports and system navigation. This integrated approach aims to improve service coordination and outcomes for individuals experiencing homelessness in the Squamish area.

The objective of this project is to prevent and reduce homelessness in Terrace by providing consistent, low-barrier access to essential supports and services through The Garage Community Space. By serving as a trusted access point, the program will also connect participants to housing supports and wraparound services, supporting pathways toward increased stability and long-term housing outcomes.

The Getting to Home Project aims to assist individuals experiencing homelessness or those at high risk of becoming homeless by facilitating access to stable housing through the Housing First Model. This initiative focuses on identifying alternative housing solutions for those who face persistent homelessness due to significant health and social challenges, which hinder their ability to secure and sustain market housing. Each participant will undergo an intake process to create a personalized Housing needs plan (HNP), thereby addressing their unique circumstances. Furthermore, to effectively tackle the issue of homelessness, it is essential for the entire community, including stakeholders and service providers, to collaborate and work together towards meaningful change.

Zero Ceiling aims to provide stable, supportive housing and comprehensive wrap-around services for 18–20 youth aged 19–29 from Metro Vancouver and the Sea to Sky Corridor between April 1, 2026 and March 31, 2027. By offering safe housing with on-site support for 12–24 months, along with case management, life skills education, employment support, and mental health services, the project seeks to prevent youth homelessness, promote stability and well-being, and empower participants to transition successfully to independent living. Through these interventions, youth are supported to achieve a healthy transition to adulthood, build meaningful relationships, and fully engage in their communities.
